The BBC has released an extended trailer and iconic picture ahead of the landmark 40th anniversary of BAFTA-winning drama Casualty, with a special double-bill airing on Saturday 5 September 2026 on BBC iPlayer and BBC One. 

Produced by BBC Studios Drama Productions, Casualty is the world's longest-running medical drama. For four decades, it has delivered powerful storytelling, unforgettable characters and ambitious television moments that have resonated with audiences across generations.

In the extended trailer, it is revealed that St James’ Hospital is the site of a major explosion that has thrown Holby into chaos. As paramedics search through rubble at the now devastated hospital, those back at Holby City Hospital must hold everything together in a large-scale rescue operation that will push some of the show’s most beloved characters to their physical and emotional limits.

Dylan (William Beck) battles personal tragedy while leading his team through their darkest hour, and Stevie (Elinor Lawless) and Flynn (Olly Rix) find themselves fighting for survival in a race against time in a tense and emotionally charged storyline that promises drama on an unprecedented scale, unlike anything Casualty has tackled before.

The anniversary episodes celebrate 40 years of Casualty while signalling an exciting new chapter, demonstrating the ambition, storytelling and emotional impact at the heart of the series. 

Melanie Hill, Barney Walsh, Neet Mohan, Charles Venn, Sammy Dobson, Kirsty Mitchell, Di Botcher, Anna Chell, Sarah Seggari, Michael Stevenson, Milo Clarke, Naomi Wakszlak and Aron Julus also star in the 40th anniversary double-bill.
